Confidential Health Appointment
All submissions are confidential and will be received by the Lemoore Union High School District's (LUHSD) Health Services Department. LUHSD offers free confidential (private) support to all students. Whether you're feeling stressed, worried, sad, angry, fighting with friends or family, have questions about sex or drugs, need to see our nurse, need to get tested for STDs, want to make an appointment at a local health center, want therapy, or want to share your goals and accomplishments with us-we are here to support you!
Education Code section 48205 requires school officials to excuse students from school to attend confidential medical appointments. The school cannot require that the student have parent or guardian consent in order to attend the appointment and cannot notify parents or guardians. Confidential appointments are appointments to receive services that minors can obtain with their own consent under state or federal law. A summary of these laws is available on www.teenhealthlaw.org.

Health Screening & Referrals
The School Nurse conducts hearing screenings as required by CA law for 10th grade students. This screening is meant to determine whether a student may be having difficulty with their hearing capabilities that may interfere with their learning process.
For the hearing screening, a device is used where headphones are placed on the student's ears and then they must raise their hand when they hear the sound being played.
Vision screenings can be conducted with a parent or teacher referral on file. This can be completed by emailing or submitting a request in person to the School Nurse. Vision screenings are usually conducted using a device that looks like a large digital camera. It scans the eyes, then prints a report if there is something abnormal detected.
If a student is screened and does not pass one or both of these assessments, a notice will be mailed home to the parents and they may also be notified by phone. It is encouraged for parents to follow up with their healthcare provider for a more thorough exam if the student does not pass.
Please note that these screenings do not replace the recommendation for students to visit the optometrist (eye doctor) once a year and the dentist twice a year for examinations and care.

Healthcare Management- Plans
Students with health conditions may have a health care plan created at the beginning of each school year and emailed to teachers. It is important that during the registration process, all health concerns are documented on the Student Health Update form or you contact the School Nurse with any new health conditions.
Some of the health conditions that may require a health care plan include (but not limited to):

Asthma
A condition in which a person's airways become inflamed, narrow and swell, and produce extra mucus, which makes it difficult to breathe.
If your child has asthma and needs an inhaler at school please complete the Asthma Action Plan and Medication Administration Request.
Allergies
An allergy is a heightened sensitivity to a foreign substance that causes the body's defense system to overreact when defending itself. The severity of an allergic reaction can vary from mild discomfort to life threatening situations.
If your child has allergies please complete the Allergy Action Plan. If your child is allergic to food please complete the Meal Accommodation Form. In addition, if specific medications are necessary, such as antihistamine or epinephrine please have your physician complete the Medication Administration Request.
Epilepsy / Seizures
Based on the type of behavior and brain activity, seizures are divided into two broad categories: generalized and partial (also called local or focal). If your child has a history of seizure activity please complete the Seizure Action Plan.

Diabetes
Students with either insulin dependent Diabetes, or Diabetes that is controlled with oral medication will meet with the school nurse and school site team annually to review and updated healthcare plans. Each school year, we require that your physician provide the school district with updated medical orders. Attached is a Sample School Insulin Orders form; however, most physician's prefer to use their own approved document.

Stock Naloxone Program
The CA Dept of Public Health has developed this 11 minute video to provide reinforcement specific to this stock naloxone program. In addition to the video link, please review the Opioid Overdose Action Plan. Please let the school nurse know if you wish to receive naloxone administration training or have any questions regarding opioid overdose and/or administration of naloxone.
Summary of Ed Code 49414.3 and the LUHSD protocol/board policy:
o Our school sites have stock naloxone to be used on any person suffering or believed to be suffering from an opioid overdose.
o Signs and symptoms of opioid overdose may include unconsciousness, inability to waken, slow or shallow breathing, choking/gurgling/snoring noises, fingernails or lips turning blue, and cold/clammy skin.
o Call 911 and the person's designated emergency contacts if known.
o Provide CPR or rescue breathing depending on the need of the situation.
o Employees and volunteers will be provided defense and indemnification by the school district for any and all civil liability when naloxone is administered in good faith and not willfully malicious intent.
o Print this indemnity acknowledgment for your own record.
o The LUHSD Nursing Department maintains the logistics of the stock naloxone program which includes the annual physician standing order for the program, proper storage, the stock of unexpired medication, and a monthly check of supplies. (Please check with the school nurse to identify where the backstock of Narcan is located)
Medication Management
"Authorization for Medication Release" forms must be updated annually. You will need to complete this form and have your student’s physician authorize the medication. Remember all medications (even those over-the-counter) require a completed form.
Once this form along with the medication is received by the health office, you will receive a receipt for the medication for your records.
*Pain Relievers: We do not keep stock Tylenol, Ibuprofen, or any other pain relieving medication on campus.
*Emergency medication such as Epi-Pen and Narcan is kept on each campus to administer to students, staff, and visitors if needed. We do NOT stock emergency medication for asthma.
